- The license number, starting with a capital 'E', can be found in the About box of EthoVision.
- The form is case-sensitive, so capital letters should be typed in uppercase!
- Do not use the last 4 letters of the license number.
- The registered user's name should be entered in the following format: [first name] [space] [last name]. Example: Donna Smith.
- The e-mail address should be the current e-mail address for the registered user.
