The form is case-sensitive, so capital letters should be typed in uppercase! Do not use the last 4 letters of the license number.
The registered user's name should be entered in the following format: [first name] [space] [last name]. Example: Donna Smith.
The e-mail address should be the current e-mail address for the registered user.
